What have you got set as your root device in the guest's config?  I
can't see any mention of what root device it's trying to use.  If you
don't already have it, in your config you should have a root="/dev/sda1
ro", I think.

Additionally, if the kernel you are using does not have the xenblk
driver compiled in (I'm not sure that kernel does), you'll need an
initrd with that driver in.
