It is actually a /dev/pts/<nr> in dom0. "xm console" will just lookup
which pseudo tty it actually is in xenstore, then connect.

No, guest linux kernel, i.e. grub/lilo inside the vm must be configured
to pass that to the kernel, exactly as you'll setup a serial console on
a physical machine.
