No, it's this:

04:00.0 Ethernet controller: Broadcom Corporation NetXtreme BCM5721 Gigabit Ethernet PCI Express (rev 11)

The device is working just fine on other systems like this and even on this system on dom0 without any domU with active eth0 or in routed networking setup.

I have a hunch that it is a problem with that particular NIC. The reason I asked is that I have a problem on systems with Broadcom rev 10-NICS (routed setup works fine though), while the ones with rev 2 works fine with bridged setup. I can't change the network card since it's a blade server. If yours is a normal (e.g. rack-mount) server, you may have better luck trying other NICs.
