You might have to use a ramdisk to boot the guest properly. It can be specified by passing "ramdisk = <name>" in the guest configuration file.

the .cfg looks like this:

disk = ['file:/xen/centos/centos.5-0.img,sda1,w', 'file:/xen/centos/centos.swap,sda2,w']
root = "/dev/sda1 ro"

I have seen some people with their root = option set to a /dev/hda1, but when i run that in the config it gives the same error but with hda1

any ideas?
