I have encountered a similar problem on one of my systems.  I narrowed it down to serial console.  The system is able to boot after I removed serial console parameter for dom0.

 

I have yet to root cause the issue.

looks like the problem is with USB.

Can anyone guess what can go wrong , did I miss some important USB configuration ?
