you have to set the ip address on the bond0 interface, set eth4 and
eth5 with no ip address at the boot process. then start bond0 wiht
your ip address. make sure your networking guys set the portchannel
correctly.
as example:

lea# sh run int gi 1/0/29

 switchport mode access
 switchport access vlan 12
 switchport nonegotiate
 channel-group 5 mode on

lea# sh run int po5

interface Port-channel5
 description etc-TX--ha1-342|drea
 switchport mode access
 switchport access vlan 12
 switchport nonegotiate
end
