We believe that this problem was solved with the following Linux
changeset:
http://xenbits.xensource.com/ext/linux-ppc-2.6.hg?cs=f0be2cc05103
which originated from Amos.
Since pushing this patch I have yet to see this issue occur.

I'm hoping to close this bug if enuff time goes by that I no longer
see it.
