Xen requires 61 bits of VA, while Vti only provides 60.  That's the main issue
my patch addresses.  The other issue is VHPT long format.  Currently Xen/VTi
allows it but doesn't use it.

It would be nice if Xen can work with any VA size.  I am thinking about how to
do that.
