Thank you for your help.
Looking at the log, something nasty is going on as you may already
noticed. It panics because no page is left, on the other hand
dom0 pages isn't reached. It's confusing.
It suggest page allocater data corruption which you mentioned before.

I tried to reproduce with the given memory map, but couldn't.
I really need your help here. Can we narrow it down?
I attached the three patches.
Apply those to xen-ia64-unstable.h and send me the boot log? 
(Or you can debug it yourself.)
