reviewing p2m table destruction patch, I found that traversing
p2m table isn't SMP-safe. this patchset tries to fix it.

With this patchset, all pgd_t, pud_t, pmd_t, pte_t are with volatile.
It's very ugly.
It strongly indicates that new types and interfaces for the p2m table
should be defined.
Although I'm not sure it should be considered,
I think that it would be a mid-term goal and post-3.0.4. not now.
