p2m exposure TAKE 2
These patches are for p2m table expsure to domain.
With this patch, p2m conversion hypercall overhead is eliminated.

CHANGES:
        updated patches following to Alex's comment. (Thanks Alex.)
        fixed a race with cpu hot plug.

Alex.
p2m_expose_init() must be called after xen_ia64_privcmd_init()
so that xxx_initcall() doesn't work. I left it as is.
