It occurs. At least we've known two instances: one is reported by 
Alex for part of EFI service, the other reported by Kan for FPSWA 
image. Both cases have code sections out of any TR-mapped areas 
on some type of boxes. Also from architectural reason, we should 
handle it.

IA64_GRANULE_SHIFT is configurable.
