Yep, I see that "port 5 still bound!" message in there and i was also
seeing bad pages. This looks like the same issue discussed in my
thread: "pv-grub boot hangs when iommu=soft. Booting kernel directly
with the flags works" :

The domU doesn't even boot, on console it reports the things below in this case
it seems to go wrong on domain creation allready.
I think port 5 refers to pci device 5 i'm trying to passthrough, without the
pci=['0000:05:00.0'] line the domain boots fine.
There is a bug in the pv-grub (or py-grub?) where extra
arguments (such as iommu=soft) choke the loader. Somebody else saw this
some time ago and as a work-around referenced the kernel and initrm
directly.
