Which version of Xen are you using?  Since we're just about to
release xeno-unstable as Xen 2.0 we're rather more interested in
bug reports on this than 1.2.

Was the media present in the drive when the domain booted?

Mounting CDs works fine for me, and I know a lot of other people
that use it too.

Supplying the output of xen_dmesg and dmesg might help track this
down.
