[IA64] fix XENMEM_add_to_physmap with XENMAPSPACE_mfn
In case of XENMEM_add_to_physmap with XENMAPSPACE_mfn,
it triggers BUG_ON(). In fact it breaks some assumptions.
Update BUG_ON() conditions.
