I have had this same issue occur on the following hardware:

HP BL490c G7
2 x Intel X5650
144GB RAM
1 x 60GB SSD

The blade was running fine for a month or two with XenServer running on it, and
recently I rebooted and the errors you have reported in this bug started
appearing and the blade would not boot further into XenServer (i.e. no SSH or
console access is working).  After several reboots the issue keeps happening.
The node is not under load at the time, as it has not got any VMs running on
when this issue surfaces.
