I can confirm too.

My configuration is
Xen 4.0.1
Gentoo x86_64 2.6.34-xen-r4 kernel

I've got 2 crashes w/o running domU.

After both problems system became frozen. 
I'm not a kernel developer... But what does `insert_pfn` function means? How
can I find where is a problem?
