Hi.  We have a problem where a domU exits and cannot be restarted.  It looks
like the problem is because its file:/blah device is already mounted.  And it
cannot be unmounted.  

Any ideas on a fix?  Or a way to umount that loop back device?

The only way we seem to be able to recover is with a dom0/host server restart.
