I'm seeing a similar problem. This is Xen-3.0.3 on i686 (compiled from source)
on an Ubuntu 6.06 host, and I'm using tap:qcow and tap:aio disks (full config
attached).

The reproduction is eerily simple:
  xm create -c error791.xen
  xm destroy vm01

"xm list" now gives the "device 769 not connected" error. When I then stop and
start xend, it bails out on subsequent "xm create" saying, "The privileged
domain did not balloon". I have not found any remedy other than rebooting to
fix this wedged state.

Please let me know if there is anything I can do to help track down this issue.
