Today, I lost /dev/sda with the same problems as the one previously described
here. I could not access any of the domains residing on /dev/sda, including
Dom0. I am increasing the severity of this defect to 'blocker'.

Last tested with changeset: 8705:690fc13948db 

The message on the console shows: scsi0 (128:0): rejecting I/O to offline
device.  There are no log messages on this one due to the fact that the I/O
device is unavailable.
