This is really a shot in the dark, but how many guests do you
currently have installed and running?
I could be making an incorrect assumption, but doesn't Xen require a
loop device to mount the iso image for installation?
So, if you're using files as storage (not block devices) and
installing from an ISO, you'd need two free loop devices, one for the
guest and one for the ISO.
If you've not changed the number of loop devices created by the
kernel, you could well be running out of them. That's all I can
really come up with that would affect this particular installation
method if it's worked in the past on this system for other guests.
