Hi,
i have the same issue with hvm guest. It looks like that the data
reading and writing is sequential. But i have no clue waht i can do.
Disk performance on the same device is under Dom0 is fine.

what are the mount options (/etc/fstab) on the DomU ?
In the xen-tools "partitions.d" example, "sync" is used, which give
poor performance.
this is my fstab (DomU)

Anything else i should provide?
