I got it working... the proper thing to do appears to be to add

fda='/var/tmp/disk_dev.img'

in your xm config file. you can leave the :floppy bit out of the disk= line altogether.

this is xen 3.0.4_1

Is there something obviously wrong with how I specify the floppy? does anyone else use the floppy drive?
