Two more items from the booted system might help - "lsmod" and "cat
/etc/modprobe.conf".

[root@hive linux-2.6.16.33-xen]# mkinitrd -v -f
/boot/initrd-2.6.16.33-xen.img 2.6.16.33-xen

That seems to not be including what your motherboard's disk controller needs.
