I had a similar problem, though not exact. Perhaps my work-around will work for 
you also.

In my case, peth0 (which I needed) would not get created, but peth1 (which was 
useless to me) was created. I tried swapping IP addresses, name designations, 
etc, but the pethX device created was always for the wrong interface.

I modified the eth1 device configuration to make it not come up on boot, and 
peth0 was being created. I also modified my rc.local script to bring up eth1, 
which worked fine with regards to xen since xen had already done its thing.

It's an ugly hack, but it did the trick.
