I've seen this happen before, not sure why. I changed 'phy:' to 'file:' and it
worked. I later rebooted and 'phy:' worked again. So you might try 'file:'
rather than 'phy:'.

Can anyone advise me what is the source of problem?
Make sure that the filesystem isn't mounted on some directory in dom0. Also,
make sure any other process isn't using it...
