Xen 4.0 recently added support for grant table mappings in the HVM P2M tables.
The current code explicitly avoids adding an IOMMU mapping in this case.
Is this an oversight or is there an architectural reason that the IOMMU is
not updated?  This seems to be the last piece needed to allow HVM driver
domains with PCI pass-thru devices to function without extra memory copies.
