This is largely the same as the RFC patch I sent out yesterday.  I've
moved the enormous inline function into c-code and cleaned things up a
bit.  For a full description of this new driver domain PCI backend and

This patch should have no impact on architectures other than ia64.  I'm
submitting it here because it lives in common code.  Keir, please apply.
