The following is an updated set of patches to roll-up everything needed
for the paravirt framebuffer to work.

Things that are different from the last submission:
* Changes to fix some little things in the xend integration that I've
noticed after using the patches pretty heavily
* Adding support for vncunused, vnclisten, vncconsole to give an even
more consistent experience between PV and FV
* Various improvements by Markus for the protocol as well as cleanliness
based on Stephen's review comments
* Fixing things so they work on x86_64 (and hopefully ia64, but I
haven't been able to test it due to not having an ia64 handy)
