Reorg patch 1 to match http://wiki.xensource.com/xenwiki/Xen3DocsToDo
First patch to reorganize the manual to match the structure in the
Xen3DocsToDo Wiki entry.
Incorporates a patch from Nivedita Singhvi to clean up the Further
Support Chapter.
