Environment:

Service Arch (ia32/ia32e/IA64): ia32e
Guest OS Type (Linux/Windows): Linux
Change Set: 20033(pcops dom0)
Hardware: NHM-HEDT


Bug detailed description:
--------------------------
We assigned 00:1a.0 or 00:1a.2 USB keyboard/mouse/disk device to guest, then
remove the real hard device and plug it in. The device does not work in guest,
but "lspci" still can see the device. We have to using "xm pci-detach" and "xm
pci-attach" command to enable it rework on guest.

00:1a.1 and 00:1d.2 has no this issue.


Reproduce steps:
----------------
1. assign 00:1a.0 or 00:1a.2 USB keyboard/mouse/disk to guest
2. remove the real USB device and plug in
3. device on guest can not work

Workaround method:
4. using "xm pci-detach" and "xm pci-attach" command to enable it rework on
guest
